Cleveland, Ohio clinic performs US’s first face transplant

A team of eight transplant surgeons in Cleveland Clinic in Ohio, USA, led by reconstructive surgeon Dr. Maria Siemionow, age 58, have successfully performed the first almost total face transplant in the US, and the fourth globally, on a woman so horribly disfigured due to trauma, that cost her an eye. Two weeks ago Dr. Siemionow, in a 23-hour marathon surgery, replaced 80 percent of her face, by transplanting or grafting bone, nerve, blood vessels, muscles and skin harvested from a female donor’s cadaver.

The Clinic surgeons, in Wednesday’s news conference, described the details of the transplant but upon request, the team did not publish her name, age and cause of injury nor the donor’s identity. The patient’s family desired the reason for her transplant to remain confidential. The Los Angeles Times reported that the patient “had no upper jaw, nose, cheeks or lower eyelids and was unable to eat, talk, smile, smell or breathe on her own.” The clinic’s dermatology and plastic surgery chair, Francis Papay, described the nine hours phase of the procedure: “We transferred the skin, all the facial muscles in the upper face and mid-face, the upper lip, all of the nose, most of the sinuses around the nose, the upper jaw including the teeth, the facial nerve.” Thereafter, another team spent three hours sewing the woman’s blood vessels to that of the donor’s face to restore blood circulation, making the graft a success.

The New York Times reported that “three partial face transplants have been performed since 2005, two in France and one in China, all using facial tissue from a dead donor with permission from their families.” “Only the forehead, upper eyelids, lower lip, lower teeth and jaw are hers, the rest of her face comes from a cadaver; she could not eat on her own or breathe without a hole in her windpipe. About 77 square inches of tissue were transplanted from the donor,” it further described the details of the medical marvel. The patient, however, must take lifetime immunosuppressive drugs, also called antirejection drugs, which do not guarantee success. The transplant team said that in case of failure, it would replace the part with a skin graft taken from her own body.

Dr. Bohdan Pomahac, a Brigham and Women’s Hospital surgeon praised the recent medical development. “There are patients who can benefit tremendously from this. It’s great that it happened,” he said.

Leading bioethicist Arthur Caplan of the University of Pennsylvania withheld judgment on the Cleveland transplant amid grave concerns on the post-operation results. “The biggest ethical problem is dealing with failure — if your face rejects. It would be a living hell. If your face is falling off and you can’t eat and you can’t breathe and you’re suffering in a terrible manner that can’t be reversed, you need to put on the table assistance in dying. There are patients who can benefit tremendously from this. It’s great that it happened,” he said.

Dr Alex Clarke, of the Royal Free Hospital had praised the Clinic for its contribution to medicine. “It is a real step forward for people who have severe disfigurement and this operation has been done by a team who have really prepared and worked towards this for a number of years. These transplants have proven that the technical difficulties can be overcome and psychologically the patients are doing well. They have all have reacted positively and have begun to do things they were not able to before. All the things people thought were barriers to this kind of operations have been overcome,” she said.

The first partial face transplant surgery on a living human was performed on Isabelle Dinoire on November 27 2005, when she was 38, by Professor Bernard Devauchelle, assisted by Professor Jean-Michel Dubernard in Amiens, France. Her Labrador dog mauled her in May 2005. A triangle of face tissue including the nose and mouth was taken from a brain-dead female donor and grafted onto the patient. Scientists elsewhere have performed scalp and ear transplants. However, the claim is the first for a mouth and nose transplant. Experts say the mouth and nose are the most difficult parts of the face to transplant.

In 2004, the same Cleveland Clinic, became the first institution to approve this surgery and test it on cadavers. In October 2006, surgeon Peter Butler at London‘s Royal Free Hospital in the UK was given permission by the NHS ethics board to carry out a full face transplant. His team will select four adult patients (children cannot be selected due to concerns over consent), with operations being carried out at six month intervals. In March 2008, the treatment of 30-year-old neurofibromatosis victim Pascal Coler of France ended after having received what his doctors call the worlds first successful full face transplant.

Ethical concerns, psychological impact, problems relating to immunosuppression and consequences of technical failure have prevented teams from performing face transplant operations in the past, even though it has been technically possible to carry out such procedures for years.

Mr Iain Hutchison, of Barts and the London Hospital, warned of several problems with face transplants, such as blood vessels in the donated tissue clotting and immunosuppressants failing or increasing the patient’s risk of cancer. He also pointed out ethical issues with the fact that the procedure requires a “beating heart donor”. The transplant is carried out while the donor is brain dead, but still alive by use of a ventilator.

According to Stephen Wigmore, chair of British Transplantation Society’s ethics committee, it is unknown to what extent facial expressions will function in the long term. He said that it is not certain whether a patient could be left worse off in the case of a face transplant failing.

Mr Michael Earley, a member of the Royal College of Surgeon‘s facial transplantation working party, commented that if successful, the transplant would be “a major breakthrough in facial reconstruction” and “a major step forward for the facially disfigured.”

In Wednesday’s conference, Siemionow said “we know that there are so many patients there in their homes where they are hiding from society because they are afraid to walk to the grocery stores, they are afraid to go the the street.” “Our patient was called names and was humiliated. We very much hope that for this very special group of patients there is a hope that someday they will be able to go comfortably from their houses and enjoy the things we take for granted,” she added.

In response to the medical breakthrough, a British medical group led by Royal Free Hospital’s lead surgeon Dr Peter Butler, said they will finish the world’s first full face transplant within a year. “We hope to make an announcement about a full-face operation in the next 12 months. This latest operation shows how facial transplantation can help a particular group of the most severely facially injured people. These are people who would otherwise live a terrible twilight life, shut away from public gaze,” he said.

Canadian inspectors to test food ingredients from China

The Canadian Food Inspection Agency (CFIA) has issued border alerts for specific protein ingredients, imported from China, that may be incorporated into products destined for human consumption.

Inspectors from the Canada Border Services Agency (CBSA) will hold products, such as wheat and corn gluten, as well as soy and rice proteins until they can be tested for melamine, the contaminant found to have sickened pets through its use in pet food. If determined to be free of melamine, the ingredients will be released to the intended recipient. Materials such as glutens and protein powders are used commonly in many forms of food products.

The CFIA said it was not acting on specific information, but rather taking a cautious approach to human protection. “That’s why we have the border lookout for the ingredient, so that we can proactively assess any potential that the product is contaminated,” Paul Mayers of the CFIA told CBC News.

Since the border alert for melamine is a new procedure, the government can’t be sure if the contaminant made it into the food chain previously. The CFIA acknowledged that the same Chinese company under suspicion in the tainted pet food affair had shipped wheat gluten to a Canadian company, which in turn used it in food for fish farms. Although the fish were subsequently eaten by people, the CFIA believes the health risk from such consumption would be low.

In related news, Canadian researchers at the University of Guelph believe they may have determined the mechanism of how melamine caused illness in cats and dogs.

Both cyanuric acid and melamine were found in urine samples from pets that died after consuming contaminated pet food. The two compounds react with one another to form crystals that may block kidney function, researchers at the university said. The researchers observed crystals formed in cat urine by the addition of melamine and cyanuric acid. The composition of these crystals matches those found in the urine of affected pets when compared by infrared spectroscopy (FTIR).

“You wouldn’t normally expect to find those compounds in pet food, and hence nobody was really looking for it,” said John Melichercik, director of analytical laboratory services. “It’s just another piece of the puzzle along the way in this particular pet-food issue.”

Libya releases four foreign journalists

Wednesday saw Libya release four journalists, detained for six weeks and, accused of entering the country illegally. The two Americans, one British, and Spanish correspondents were taken to Tripoli’s Rixo hotel upon their release.

James Foley, Clare Morgana Gillis, Nigel Chandler and Spanish photographer Manu Brabo were captured on April 4 by Muammar al-Gaddafi’s military; the day prior to their release, a suspended sentence of one year and $154 fine were imposed for their illegal entry to Libya.

Moussa Ibrahim, a government spokesman, said the detainees are free to stay and carry on reporting in Libya or, if they requested to leave, could be escorted to the Tunisian border. Claiming their detention was due to confusion over the journalists’ identities, and citing foreign combatants fighting with rebel forces, as the cause of the mix-up, Ibrahim offered an apology and stated that, as journalists, they are harmless.

The journalists worked for The New York Times, GlobalPost, the Atlantic, USA Today, and the BBC. The fate of South African photographer Anton Hammerl is still unclear and he is believed to be still missing.

The foreign journalists were held in different detention centres around Tripoli and refused contact with the outside world.

The conflict between uprising rebel forces and Gaddafi’s Libyan government continues from February this year.

Under the UN, NATO forces are currently carrying out air strikes across Libya recently focussing attacks in Tripoli. Aiming to protect civillians from Col Muammer Gaddafi trying to quash the anti-government rebellion.

Book Fair 2.0; On bloggers, ebooks and pirates

The Internet is very much present at the Frankfurt Book Fair 2007, not just, like in previous years, as a means for the journalists who have 80 workplaces for their own notebooks to report on the fair, but like before as a chance – and as a threat for rights-owners of digital media.

After a marginal existence in the previous year, bloggers have got their own “living room 2.0” at the fair, furnished with everything a blogger needs, including media attention. Every day from Wednesday October 10 to Sunday October 14 they will write and podcast about the big names to meet, the events not to be missed and their very personal experiences and thoughts. Three of the bloggers write in English, two English language podcasts are done, to widen the reach of the Book Fair 2.0. The blog entries and podcasts will be available until after the book fair at http://www.book-fair.com/en/wordpress/ and the bloggers themselves can be visited on the weekend at hall 4.2, Q411, though until now it is more the media and less the visitors of the fair, the bloggers come in contact with.

Digitalization and digital media, especially books and magazines offered digitally, are a hot topic at this year’s Frankfurt Book Fair, as more and more publishers want to see the digital counterparts of their traditional media not just as a field to be present in, but as a possible profit center. With scientific books, this move already was quite successful: Publishing house Springer for example, offering over 40,000 ebooks and over 1700 electronic magazines, of which over 1200 are still actively continued with Springer, nowadays does an ebook-variant of every traditional scientific book they print – and already has the largest part of their cash-flow from digital media.

This is harder for fiction publishing houses as the Pabel Möwig group (VPM), which has become active early. They do offer the digitized new adventures of – say, the outer-space-hero Perry Rhodan -, but the turnover is still only a small addition to the print and other media versions. Readers become readier to read on a screen, but their readiness is still growing slowly. Since a new generation of readers is growing up using the internet as a reference work – especially Google and Wikipedia – it will become more and more natural in the future.

A growing number of service companies in the publishing sector therefore offers re-digitalization apart from increasingly effective content management systems, with which new forms of media can easily be compiled from the contents of a data base.

Older works, of which the publishing house owns the rights, but for which a reprint might not be profitable, are scanned, divided into content sections and tagged. When the original type face isn’t good enough, books are typewritten in third world countries two or three times which are corrected and merged into a final version. Once in the system, digitalized books can be at disposal as MobiPocket ebooks or Print On Demand (POD) and with aid of the Amazon BookSurge program remain available, possibly even within 24 hours.

Digital content can also be used as a marketing-tool with the “Search Inside” from Amazon.com, where the full text of a book is visible but only small parts of the book are shown at a time.

Right after Amazon, Google also presented their own projects for the digitalization of books, where publishers have the option of just sending a box or container full of their books in printed form and leave the job of digitalization to Google, where afterwards their content will be findable with Google Book Search. The difference between those two internet services was obvious, though: Amazon wants to earn money with books, while Google’s business is advertising, their revenue model is AdSense and AdWords, targeted as perfect as possible with full text search. Both services had to answer questions as to how they will protect the content from unpaid exploitation, as probably fewer and fewer users will be willing to pay for a digital eBook when they can read the content for free, up to twenty pages at a time. The freeloader mentality of many Internet users was seen as a threat by many of the publishers.

The science includes names of birds, animals, their sounds, trees, flowers, leaves and many similar things. These can be taught by showing them and making them feel. Younger kids can enjoy their classes for identifying colors. Use various things in front of the colors. Like use tomato in front of red color and so on. There are toys which are specially designed for colour identification. These sets comprise of various toys like apple, flowers, ball and many more. Children can learn about the colors, names of the things and shapes simultaneously with the help of these toys. Shapes like square, circle, rectangle, triangle and oval can also be learned from the various toys. Learning the alphabets can be fun when presented with colors or things. For example, if the letter is A, then show an apple with it. Similarly, kids will remember the alphabets as B for ball, C for Cat till Z for Zebra. Some of the things are difficult to show in real. Then show the pictures of these objects. There are books available in market, which have alphabets printed along with the pictures. Once the alphabets are known by heart, you can teach them how to write. For this purpose, you can use the boards which have the alphabets printed on them. Kids can practice writing on these boards. Writing can be made more fun by providing the kids with colors and blank board. Children love to use colors and drawing. Use their liking for teaching them how to write. To teach the words and their meaning, you can take help of toy dictionaries. These dictionaries comprise of buttons with printed alphabets. When buttons are pressed, letters are displayed on the screen on the dictionary. You can also get talking dictionaries. These dictionaries talk the entered word. If any word is wrongly spelt, then it automatically corrects it. Separate section of the dictionary tells words with similar pronunciation, but with different spelling and meaning. You can get some audio and video CDs for fun learning process. These CDs comprise of various songs and visuals which are made especially for kids. The downloadable software can help the kids learn about colors, counting, shapes, sizes and identifying time. Thus, there are many toys, books and visual aids which make the learning process fun for kids. Use these methods to teach kids and they’ll love it, they won’t even realize that they are learning something, since it’s so much of fun.

Belgian bus company knows solution for car parking problems

Do you have a hard time finding a parking space? Take the bus or tram if you go to the city. That’s the message Belgian bus company De Lijn (The Line) is sending to promote public transport as a solution for car parking problems. As a part of their media campaign, they have jokingly suggested that people use the top of the busses as parking space.

Another idea they are using in their media campaign: maybe you could park your car on the bottom of a canal? The bus company is using an invented diving company called Cardive, which has divers that offer to dump your car in the canal. The divers walk around in cities and hand out free bus tickets and maps of the bus network.

To reach car drivers who are not using public transport, the media campaign has several radio commercials that present other solutions to the car parking problem. You could use “asphalt-spray” to camouflage your car, making it invisible for policemen (although you then need to remember where you’ve parked). Or you could use the “flat tire kit”, which comes with a fake flat tire and an inflatable dummy, so it looks like you’re replacing your flat tire. The final idea the bus company has is to use a View-Master to fool parking guards into believing that your car has been stationed correctly.

A survey among 4000 customers of De Lijn shows that two out of three car owners who use public transport, do so to avoid parking space troubles, and in cities this percentage rises to 90%. The survey further shows that 39% use the bus to go shopping.

During the month of May, the auto-bus and the divers tour several cities in Flanders (Leuven, Hasselt, Ghent, Bruges and Antwerp).

Wikinews will be attending The 2007 MuchMusic Video Awards this weekend, a popular annual event in Toronto, Ontario, Canada. True stars will be out to play, present, and else wise schmooze at the CHUM-City Building just above the city’s Entertainment District in the Queen Street West neighbourhood.

MuchMusic is the most popular music channel in Canada, and has been holding the event since 1990. Roughly 6000 fans line the streets surrounding Much headquarters each year, and 1200 more score “the wristband” and enjoy a free festival-style show in the parking lot, watching four outdoor performance areas spread out in the downtown location. New this year is a special roof-top stage, on the top of the building.

Last year’s show reached 3.5 million viewers in Canada and 100 million around the globe, with broadcasts in 65 countries.

Performing at the show will be Avril Lavigne, Fergie, Billy Talent, Hilary Duff, Alexisonfire, Maroon 5, Belly, The Used, and Finger Eleven.

On stage presenting will be Nickelback, Jay Manuel (Canada’s Next Top Model, America’s Next Top Model), Tara Reid (American Pie, this fall’s Land of Canaan), Joss Stone, Sum 41, Amber Tamblyn (Joan of Arcadia, Grudge 2), Hedley, Chris Bosh (Toronto Raptors), Sean Avery (New York Rangers), George, Sam Roberts, Emilie de Ravin (LOST), Marianas Trench, and Kardinal Offishall.

Photographer Robin Wong will be photographing the red carpet of the MMVAs for Wikinews and Wikipedia. He first helped the sites in April of this year, photographing Hilary Duff at MuchMusic. Iranian International Master Dorsa Derakhshani discusses her chess career with Wikinews

In February 2017, the Iranian Chess Federation announced two teenage chess players, Dorsa Derakhshani and her younger brother Borna Derakhshani, were banned from representing the national team. The federation announced their decision although Dorsa Derakhshani had previously decided and informed the chess federation she did not wish to play for Iran.

Dorsa Derakhshani is currently 21 years old and holds the International Master (IM) as well as Woman Grand Master (WGM) titles. Her brother, Borna, plays for the English Federation and holds the FIDE Master title.

Dorsa Derakhshani was banned since she did not wear a hijab, an Islamic headscarf, while competing at the Tradewise Gibraltar Chess Festival in January 2017. Under the laws of Islamic Republic of Iran, hijab is a mandatory dress code. Her brother Borna Deraskhsani was banned for playing against Israeli Grand Master (GM) Alexander Huzman at the same tournament. Iran does not recognise the existence of Israel, and previously, Irani athletes have avoided playing against Israeli athletes.
